Samaritans can help with brokerage funds to support young people at risk of homelessness through our Homeless Youth Assistance Program (HYAP) and Family Outreach Child and Adolescent Service (FOCAS) programs.

HYAP

The Homeless Youth Assistance Program (HYAP) is a brokerage and support service for the community sector supporting young people aged 12-15 years. HYAP is available to support young people who are connected to a Specialist Homelessness Service (SHS) in the Hunter and New England Area Health Districts.

How it works:
HYAP is available to support young people who are at risk of homelessness, disengaged in education, have Mental Health, impacted by DV, trauma and/or family breakdown, misuse of drugs and alcohol and interaction with the justice system and Out of Home Care (OOHC).

Samaritans Family Mediation and Counselling
Samaritans HYAP can also provide accredited family mediation services and counselling with a social worker.

How to access HYAP

Samaritans HYAP can assist young people aged 12 - 15 who are at risk of homelessness. Young people must be supported by a Case Manager.

A service can apply for brokerage on behalf of a young person at risk in the following ways:

SHS Services can make a direct application to HYAP via CIMS referral.
Non-SHS Services must engage with a SHS Service in order to apply for HYAP brokerage on behalf of a young person. This is so that a young person can have access to a Case Manager.
